Practice Setting

• Outpatient clinic focused on Medicare patients (65+)
• Located in a diverse, Spanish-speaking community
• Team includes NPs, RNs, MAs, and a scribe
• Strong support in a collaborative care model

Schedule

• Monday through Friday, 8a-5p only
• No weekends or holidays
• Predictable weekday routine; great for work-life balance

Cases and Procedures

• No procedures required
• Focus on chronic disease management: diabetes, CHF, HTN, CAD, mild cognitive impairment

Requirements

• MD or DO, board-certified in Internal Medicine or Family Medicine, or Certified Family Nurse Practitioner
• Active Illinois license and DEA
• Must be fluent in Spanish
