Requirements

Requirements:

• Current California State NP or PA license
• Current Nurse Practitioner Furnishing Number (for NPs)
• Nurse Practitioner furnishing number of NPs (for NPs)
• CAPA Controlled Substance Certification (for PAs)
• Active DEA registration
• Must reside in one of the following approved states:
  • California, Oregon, Washington, Arizona, or Florida

Preferred:

• 6 months or more of related clinical experience in primary care
